Treatment cataracts without pain

Decrease sight is an early symptom of a cataract. Thus even correctly chosen glasses don't lead to a sight complete recovery. What to do, if it happens? How to restore the lost sight? The leading expert, ophthalmosurgeon, the candidate of medical sciences Arseny Aleksandrovich Kozhuhov tells.

Cataract is named turbidity in an eye therefore there is a gradual loss of sight. More often this disease is age, that is develops as consequence of the general process of aging of an organism. Meets congenital and traumatic cataracts less often.

Development of this disease is influenced by many factors: infringement of a metabolism and an avitaminosis, a psychological overstrain, traumas, environmental contamination, surplus of sunlight, radiation. Usually the cataract develops on both eyes though one eye can be amazed for some months (or even years!) before another. The main signs of disease are decrease's sight, visual acuity decrease afar and close which doesn't manage to be corrected by means of points. In the started cases of eyes can distinguish only light.

Unique way of treatment of a cataract – surgical intervention.

Be careful!

Now in the world pharmaceutical market there was a great variety of different medicines in tablets, eye drops, elixirs and other forms which, ostensibly, are capable to cure a cataract. Many of these medicines are widely advertized in mass media, as the best, or, even, unique means from a cataract. Thus cost of many from them is very high. There were even special devices and special points which, under statements of their authors-inventors, are capable to relieve the whole world of a cataract without operations.

However to experts-ophthalmologists it well-known that actually, any therapeutic medical actions (instillations of drops, tablets, injections, etc.) can't cure a cataract, of them can slow down some some process of progressing of disease delay the operation moment.

So the person that would be desirable it to trust in a wonderful medicine which necessarily should help is arranged, and it is desirable from all illnesses at once. Thanking this feature of human mentality plus to the big investments in advertizing, also the huge market of new "miracle" medical products from all illnesses has expanded today.

Effective ways of treatment of a cataract

The first mentions of surgical treatment of a cataract meet in the ancient Indian manuscript, a dated fifth century B.C. The method described there – reclination – consists that aesculapius by means of a needle pierced a cornea and displaced a muddy crystalline lens downwards in vitreous body, providing with that a transparency of optical environments. Even now such procedure is applied in some undeveloped countries at impossibility of application of more modern ways of treatment.

At simple removal of a muddy crystalline lens, for rather normal sight additional correction by means of very thick points – over +12 dioptries is required. Therefore modern methods of treatment of a cataract are based on replacement of the grown turbid crystalline lens with the artificial. The most perfect, for today, a method of removal of a cataract and the most widespread in the developed countries of the world is the method phacoemulsification. In our country another is most popular – extracapsular extraction cataracts.

Extracapsular extraction cataracts

Till now in Russia the classical way of extracapsular extraction cataracts still is most extended. This way means the big wide cut (10-12 mm) through which the kernel of a crystalline lens and lenticular weights leaves. The lenticular bag remains untouched. After removal of the grown turbid crystalline lens the artificial is implanted into it. During carrying out of the given operation the crystalline lens kernel leaves so that to leave a back capsule of a crystalline lens on a place. The surgeon deletes a kernel the uniform block. The artificial crystalline lens can be put both in back, and in the forward chamber of an eye, and is kept by flexible loops or short moustaches. This operation allows to keep appreciably a correct anatomic structure of an eye, but, because of the big cut, an extensive zone of intervention and imposing of the big seams, demands the long rehabilitation period and often leads to development of a high postoperative astigmatism as seams pull a cornea. The postoperative period is accompanied by long rehabilitation and necessity to remove seams in terms from 3 months till 1 year after operation. Operation is carried out at first on one eye, and only after a while on other that the patient always could use at least one eye.

Unfortunately, despite all these lacks, such operations are still carried out in the majority of ophthalmologic clinics of Russia, including in Moscow. It occurs, first, because of absence in these clinics of the perfect equipment and toolkit and, secondly, because of technical simplicity and traditional character of these operations. We in the practice have departed for a long time already from extracapsular extraction cataracts and we apply to cataract surgery only small self-pressurized cuts.

First of all, it is operation – phacoemulsification cataracts which we carry out in all cases of surgery of a cataract.

Phacoemulsification

By most not traumatic and a safe method for today all over the world it is recognized phacoemulsification. This technique for the first time offered still in the sixties American ophthalmosurgeons by Charles Kelmanom, has passed further a fast way of development and now is the most popular all over the world. And now it wins the Russian ophthalmology more and more. In the most progressive ophthalmologic clinics of our country experience of carrying out of such operations makes already more than 15-20 years.

Phacoemulsification consists at a distance cataracts by means of ultrasound through very small self-pressurized micropuncture (2,5 mm). Through this micropuncture the ultrasonic probe is entered into the grown turbid crystalline lens. Destroyed by ultrasound to a condition of an emulsion the substance of a crystalline lens is sucked away from an eye by the special device – phacoemulsificator. During carrying out of the given operation the crystalline lens kernel leaves so that to keep its capsule. After that through the same micropuncture the flexible turned off artificial crystalline lens is entered into a lenticular capsule with memory of the form and kept there by flexible basic elements.

Anesthesia only in the form of drops, without pricks and without a pain is thus applied. Such operation is absolutely painless, doesn't provoke development of a postoperative astigmatism and provides fast anatomic and functional restoration of an eye. The micropuncture is so small that is pressurized independently, right after the operation termination. Therefore procedure doesn't demand suture and reduces to a minimum danger of occurrence of complications in hands of the skilled surgeon.

Despite so difficult manipulations for the uninitiated person, this operation lasts all about 10-15 minutes and is spent is out-patient under a local (drop) narcosis. In 1-2 days after operation it is possible to renew visual loadings: a new view to watch TV, read, write, sew, bathe and to sleep (in any position).

Thus, phacoemulsification possesses many advantages:

  • Safety and painlessness;
  • Fast rehabilitation;
  • Absence of long restrictions of visual loadings;
  • Achievement of high visual acuity in short terms;
  • Absence of a postoperative astigmatism;
  • Absence of necessity to delete seams.

For today the method phacoemulsification extends in the world and in Russia more and more. However advantages of this method are shown only at a combination of several important conditions: exact observance of all technological stages of operation, use of the high-quality expensive equipment, tools, expendable materials and artificial crystalline lenses and high qualification of the surgeon. All it are presumed only by provided enough clinic, very high-class.

In Open Society "Medicine" Clinic all conditions and requirements for successful carrying out of operation are met. For carrying out phacoemulsification we use one of the most perfect devices - phacoemulsificator Millennium, manufactures of company Bauch and Lomb (USA).

Proceeding from all stated clearly that the cost price of such operation and, hence, its cost is high enough. However in this case the reached purpose justifies the spent means.

In the international center ophthalmic surgery and laser correction of sight of Clinic of Open Society "Medicine" on technology phacoemulsification operations on cataract treatment in 100 % of cases are performed.

Artificial crystalline lenses

Separate theme for conversation are artificial crystalline lenses which are implanted into an eye after cataract removal. Today in the world there is a large quantity of the most different models of crystalline lenses and firms, their making. Crystalline lenses happen rigid and soft (developing). At use of technologies of small cuts, such, as phacoemulsification, it is expedient to implant only soft, developing crystalline lenses to spend implantation through the same micropuncture through which the cataract has been removed. In case of implantation of a rigid crystalline lens, a puncture to have to increase to 5-6 millimetric cuts, thus suture besides is required. Because of this advantage of operation are partially lost.

Success of the performed operation in many respects is defined by quality of a material and quality of a surface of an artificial crystalline lens. For manufacture of the most quality crystalline lenses special expensive technologies are required. For today there are some the most known world manufacturers of crystalline lenses. The majority ophthalmosurgeons recognize that the most qualitative though also, crystalline lenses of line Acrysof of manufacture companies Alcon (USA) are expensive. These crystalline lenses have ideally smooth surface and don't damage a fabric, have a small thickness that allows to implant them through a micropuncture. They are executed from biologically inert material and consequently don't cause an inflammation, on all life remain ideally transparent and as provide the maximum protection of a retina against destroying action of solar ultra-violet beams.

Last working out of company Alkon – crystalline lenses Acrysof Restor is unique. In such crystalline lens there are various focal lengths both for "distance", and for "near" that allows to receive after operation as much as possible high visual acuity afar and close without points. Application of such crystalline lens for active people of able-bodied age is especially important, life and which professional work is connected with periodic change of distance to considered subjects (the book text, the computer, driving of the car, playing sports and so forth) and for those who can't or doesn't want to wear glasses.

Implantation of crystalline lens Acrysof Restor relieves our patients of necessity each time to catch at points. Unique properties of these crystalline lenses allow us to use them, now, not only for the patients, suffering a cataract, but also for treatment of patients with an age far-sightedness (so-called presbyopia). Carrying out of such operations demands most quality and accuracy of the diagnostic equipment, and as accuracy of calculation of necessary optical force of an artificial crystalline lens. Specially certificated ophthalmologic clinics and surgeons-ophthalmologists have the right to implantation of such crystalline lenses only. We have such certificate. In the international center ophthalmic surgery and laser correction of sight of Clinic of Open Society "Medicine" we use such crystalline lenses at surgical treatment of a cataract. It allows our patients at any age accurately to see world around in bright paints both afar, and close.

Cataract (from greek. Katarrhaktes – falls), the cataract of an eye interfering passage of rays of light in an eye and leading to decrease of visual acuity. The term the cataract reflects wrong representation of ancient Greeks on which the cataract reason is outpouring of a muddy liquid between an iris of the eye and a crystalline lens.

Good advice

Some substances warn occurrence of a cataract or slow down its development.

Medical researches have established that people who consume vitamin C a little, are in a greater degree subject to cataract development. This fact concerns a vitamin C role as antioxidant. Even if the cataract has already appeared, vitamin C can slow down its development and improve sight at reception of vitamin of 500 mg once a day.

Vitamin E is an other strong antioxidant, also plays an important role in protection of a crystalline lens against a cataract. Begin with a dose in 100 mg and continue thus within one week. But remember that at many people vitamin E increases a blood pressure.

Selenium is a microcell necessary for an organism for synthesis own powerful decontaminant of free radicals, glutathione which protects a crystalline lens from age changes. Accepting a small dose of selenium, it is possible to lower vitamin E consumption.

Beta carotin, the predecessor of vitamin A, also protects eyes from oxidizing processes and age changes. As beta carotin not so is toxic, as vitamin A, by-effects are less probable.

Deficiency of zinc promotes formation of a cataract at laboratory animals (including fishes), and also at the person. Zinc consumption has special value for older persons which often suffer insufficiency of this microcell.

The diet rich with simple sugars, can promote cataract development. It is necessary to reduce the sucrose use. Necessary for you of sugar should arrive mainly from fresh fruit and dairy products, and even these sugars it is necessary to limit total so that they made 30-50% of all consumed carbohydrates.
